For the convenience of the club secretaries and the District Secretary
and District Membership Chairman, the following fill in forms are available.
Click on Monthly Attendance Report to access form. Just type in the information. If corrections are necessary, click on
Reset. When all the information is correct, click on Submit. This will
automatically send the information to the District
Secretary and give you a receipt.
